A fall is an unplanned event that usually ends with a person on the floor. Falls can be fatal or non-fatal. They also have the ability of major harm such as, head trauma, lacerations, broken bones and, internal bleeding leading to increased healthcare utilization.
Falls can be caused from medication side effects, age related loss of muscle mass, vision loss, symptoms of illness, drop in blood pressure and/or gait imbalance. Injuries tend to be more common and lead to more serious affects as people age. A serious fall in an elderly person can lead to serious injury, long term skilled nursing care, or even loss of independence.
At Bloom Health Services, we complete a comprehensive fall risk assessment during your initial enrollment assessment. In event that you or your family member's health or medication status change, we will then complete an updated fall risk assessment to keep your care plan current and to ensure quality service and care.

Once initial assessment is complete all gathered information will be placed in your individualized care plan This is updated annually, if there are any major medical changes before the annual assessment, we will update your care plan accordingly.
All information gathered from the care plan will be collected. A licensed nurse will review information, supply you, your family, representative, or service coordinator with recommendations of changes that may need to be made in your homes to prevent falls. We will then formulate a guide for our caregivers to follow while providing home care services. This guide is detailed for each individual consumer. Diet changes maybe suggested at this time.
